The app is called Account Status.
The problem is on the very first page tab labeled Main Menu. I was attempting to circumvent the "Call properties" using an ANI lookup by entering in my own 10 digit phone numbers. I have 5 entries in the "ani" table under the column labeled "phone". When I enter my 10 digit mobile number I have NO results returned. When it routes me to the 16-digit look up for the "account" table database I have NO results returned.
So to me, it is like I have no connection with the tables when trying to get results on matches that I know exist.

Hi Dave,
You are not the owner of "Account Status (copy)", it was shared to you by a member of our sales team. That's why you do not have access to your Simple Database via that app. You should clone the application, so you will own a copy of your own. You will then be able to access Simple Database through your own copy.
